I agree that under no circumstances will I put a stop payment on these charges or cancel my credit card.
 
I further agree that if for any reason, including any action or request made by me or on my behalf, this charge or any part of this charge is refused, contested, or declined, or otherwise not paid, I am and will be personally liable for the entire charge, including interest at the rate of 12% per unnum until paid, and including court costs and attorney fees. I further agree that venue and jurisdiction in any action upon this authorization and these agreements shall be in the superior court of Washington in and for King County.

For security purposes and to ensure the payment is linked to the correct move, the cardholders name must match the name printed on your Bill of Lading (contract).
